A skull is a trinket placed in one's inventory. Skulls are won by all players still alive when a player team beats a team of skellies.
Skulls can be held in portraits[1], and they can be traded in for weight loss concoctions and skelly dolls at the trading post. Skulls do not decay and can be displayed as trinkets.

Historical notes
Skulls were untradeable until release 2005-10-20. Also, a player could only have one at a time. Now, there is no known limit.
Skulls were implemented in release 2005-02-11.
